What to take to the WIC office for your first visit:

Before you visit your local WIC office for the first time, you should gather the following items and take them with you.

  • Proof of identity for each applicant (ex. Driver's license, Social Security card, birth certificate or baby's hospital record)
  • Proof of income for everyone who works in the household (ex. Paycheck stubs, income tax return, or Medicaid or Food Stamp ID card)
  • Proof of Pennsylvania residency (ex. Driver's license, utility bill, or recent mail)
  • Bring immunization record for all children up to two years of age
  • Bring children under age 5
  • If applicable, proof of pregnancy from your doctor stating your due date

WIC does not require proof of citizenship or alien status.
